The HHA certification must be renewed every two years from the original date of issue. To renew the HHA certification, you must submit:

  • Fill out and submit the Renewal Application ODH Form 717 to the Oklahoma Nurse Aide Registry.
  • Submit the $10.00 renewal fee.
  • Employment Verification – Must submit proof of 8 hours of paid work in nursing or nursing-related services during the 24-month certification time frame.
